Sonographic features of a tophaceous nodule.

چکیده
• Vol 7 • November 2005 746 Subcutaneous nodules with prominent inflammatory features are a diagnostic challenge [1]. The differential diagnosis includes ganglion, cyst, space-occupying lesion, tenosynovitis, rheumatoid nodule, infection, and tophaceous nodule. An imaging technique that could specifically demonstrate monosodium urate deposits within tissues can be extremely helpful, especially in patients presenting with swelling of soft tissues or nodules of unknown causes. On Doppler ultrasonography, the tophi appear as attenuating hypoechogenic structures with shadowing and hypervascularity representing inflammation, surrounded by hyperechogenic tissue [2]. These findings cannot be considered specific for gout as they resemble rheumatoid nodules, but are different from those of abscess or fluid collection [2]. We describe a patient who developed a painful, red, warm swelling above the second and third metacarpophalangeal joints. The Doppler ultrasound (SonositeTitan, Linear transducer L38 with a frequency 5–10 broad band MHz) examination allowed us to differentiate the lesion from an abscess or joint effusion, and to suggest gouty tophi.

Thyroid Tuberculosis Presenting as a Cystic Nodule
Although tuberculosis involves every organ and has a wide range of clinical manifestation, however involvement of the thyroid gland is rare. We report a case of a 25-yr-old female with supraclavicular lymphadenitis that developed nodular swelling of the thyroid and had symptoms and sonographic features mimicking a nodule with a cystic component. The diagnosis was made by using fine needle aspir...
متن کاملNew sonographic criteria for recommending fine-needle aspiration biopsy of nonpalpable solid nodules of the thyroid.
OBJECTIVE The purpose of our study was to provide new sonographic criteria for fine-needle aspiration biopsy of nonpalpable solid thyroid nodules. MATERIALS AND METHODS Sonographic scans of 155 nonpalpable thyroid nodules in 132 patients were prospectively classified as having positive or negative findings. Sonographic findings that suggested malignancy included microcalcifications, an irregu...
متن کاملAssociations of the BRAFV600E Mutation with Sonographic Features and Clinicopathologic Characteristics in a Large Population with Conventional Papillary Thyroid Carcinoma
OBJECTIVE To evaluate the association of the BRAF(V600E) mutation with sonographic features and clinicopathologic characteristics in a large population with conventional papillary thyroid carcinoma (PTC). METHODS We retrospectively reviewed the sonographic features, clinicopathologic characteristics, and presence of the BRAF(V600E) mutation in 688 patients who underwent thyroidectomy for conv...
متن کاملSonographic scoring of solid thyroid nodules: effects of nodule size and suspicious cervical lymph node.
INTRODUCTION Ultrasound is the most frequently used imaging method to evaluate thyroid nodules. Sonographic characteristics of thyroid nodules which are concerning for malignancy are important to define the need for fine needle aspiration biopsy or open surgery. OBJECTIVE To evaluate malignancy risk of solid thyroid nodules through sonographic scoring. The effects of nodule size ≥2cm and asso...
متن کاملSignificance of ultrasound features in predicting malignant solid thyroid nodules: need for fine-needle aspiration.
OBJECTIVE The purpose of this study was to provide sonographic and colour flow criteria helpful for differentiation between benign and malignant solid thyroid nodules. METHODS This prospective study was carried out at Sindh Institute of Urology and Transplantation (SIUT), Karachi Pakistan from 01.05.07 to 31.12.08. Sonographic scans of 78 thyroid nodules in 66 patients were performed and char...
